Consent Form

Although every precaution will be taken to ensure your safety and well-being before, during and after your appointment, please be aware of following information and possible risks:

• There can be some risk of irritation/allergic reaction to the treatment. any sensitivities and allergies must be disclosed to your technician prior to your appointment. 
• Additional conditions could occur or be discovered during the treatment, affecting the your ability to tolerate the treatment. 
• Clients consent to "before and after" pictures for the purpose of documentation, potential advertising and promotional purposes. 

when booking an appointment with Bare Faced Beauty Studio, the client agrees to the terms and condition of the company and thereby gives permission to the therapist to perform the procedure discussed or booked. The client agrees to hold the therapist harmless and nameless from any direct or indirect liability that may result from the treatment. Clients agree that they have disclosed any known allergies or prescription medication prior to the commencement of the treatment and therefore accept all responsibility. Whilst the technician will take every reasonable precaution to minimise or eliminate negative reactions as much as possible, in the even that a reaction occurs, the technician is not liable for an allergies or reactions that occur as a result of the treatment. Clients agree to the procedure and accept any direct or indirect risks, therefore removing the liability of the technician and Bare Faced Beauty Studio. 

Cancellation Policy:  Here at Bare Faced Beauty Studio,  we understand unforeseen events happen and you may need to cancel an appointment. We ask that you give us 24 hours notice so we can adjust our schedule to accommodate other clients and inform our service provider. If you cancel within a 24 hour period, you will be charged a $20 non refundable cancellation fee

No Show Policy:  If you are a no show to your appointment, any following booking will require a $20 non-refundable deposit in order to secure your next allocated appointment time.  

Late Policy:  If you are running late to an appointment we ask that you let us know,  otherwise we will assume you will not be showing up and may release the appointment. If you are less than 15 minutes late we will continue with your appointment, however please note that the end results may not be as promised to do a reduced timeframe. If you are more than 15 minutes late, If you are more than 15 minutes late we will try our best to accommodate you but reserve the right to apply the no show policy. Please keep in mind we will always do our best to accommodate your session.
